Coaching Agreement
Hello and welcome! I’m delighted you’re here to begin your journey toward your vision of good health and wellbeing.
The coaching program (the “Program”) you are enrolling in is outlined in the services description on the website. This Agreement sets the foundation for our work together.
TERMS OF COMMITMENTS
As a Coach, I acknowledge and respect the Client’s role as the expert in their own life, based on personal preferences and lived experiences. I commit to:
Create an open, heart-centred, collaborative space for deep reflection, creative thinking and change-making
Hold the vision for what’s possible for your goals, wellbeing, and growth
Support you through any roadblocks or challenges that may arise
Provide honest, kind and constructive feedback as needed
Honour your decisions and processes
Encourage accountability as needed
Treat your information with care and confidentially
Be prepared, on time, and fully present for all sessions
As a Client, you acknowledge that the Coach’s role as a facilitator and they do not direct, advise or instruct the Client what to do. You commit to:
Take sole responsibility for your actions, decisions, and behaviours
Set the agenda and direction based on your goals and desired outcomes
Be open-minded, receptive, and respectful of the coaching process
Make conscious choices that reflect and align with your values, needs and goals
Only agree to actions and plans that genuinely feel right for you
Implement strategies or tools that feel supportive between sessions
Share what’s true for you – even when it feels challenging – this helps us work together effectively in service of your highest good
Be prepared, on time, and fully present for all coaching sessions
COACHING CONTRACT
Health coaching is a client-led, goal-oriented process that supports individuals to make positive lifestyle changes. It is grounded in trust, curiosity, and care, providing a confidential and non-judgmental space for the Client to engage in self-discovery, developing insights and taking meaningful action.
The Client acknowledges full responsibility for their life and physical, mental and emotional health and wellbeing, including that of their family and dependents (where applicable), as well as all decisions, actions and results arising during and after the Program. As such, the Client agrees that the Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Coach.
The Client understands that:
The Coach is trained in health coaching and behavioural change and serves as a guide to help clients make positive lifestyle changes toward reach their health goals
The Coach does not provide medical, dietetic/nutritional, or allied health services; diagnose, treat, or cure any disease, physical or mental ailments; or prescribe, assess or adjust prescription medications, therapeutic meal plans, or dietary/nutritional supplements
The Coach does not act in the capacity of a doctor, pharmacist, dietitian, nutritionist, psychologist, counsellor, or other licensed/registered health professional, and the Program is not intended to replace services provided by these professionals
Any tools, resources or suggestions shared by the Coach are for educational or informational purpose only – they are not meant to substitute licensed medical or healthcare services outside of the Program
The Client agrees to consult their healthcare providers before making any lifestyle, dietary, or treatment/medication-related changes. The Client expressly assumes the risks of the Program, including the risks inherent in making dietary and/or lifestyle changes.
The Client releases the Coach from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the Client ever had, now has, or will have in the future against the Coach, arising from the Client’s past or future participation in, or otherwise with respect to, the Program, unless arising from the gross negligence of the Coach.
Fees and payment:
The Program fees are outlined in the services description on the website.
Scheduling and Cancellation:
Coaching sessions will be scheduled at mutually agreed times and locations.
All coaching sessions (including rearranged sessions) must be taken within the agreed timeframe specified in the services description on the website.
If you need to cancel or reschedule an appointment, please give at least 24 hours of notice. Any missed sessions without notice will be forfeited.
Repeated cancellations or rescheduling may impact the effectiveness of the coaching process, and this should be addressed by the Coach and Client as soon as possible.
The Coach reserves the right to cancel the Program if at any point the Coach feel it’s not appropriate or beneficial for the Program to continue. Any unused sessions will be refunded pro-rata.
Intellectual property:
All materials provided during the services are for your personal use and remain the property of the Coach.
Unless agreed otherwise, please do not share content with others unless explicitly permitted.
You are welcome to share your personal experience with the Program publicly.
Confidentiality:
All personal information shared will be kept confidential, except where disclosure is required by law.
Your name and identity will not be used in any marketing materials without written consent.
If you choose to publicly share your participation in coaching, confidentiality obligations regarding the relationship may be considered waived – however your information will continue to be kept private unless you explicitly agree otherwise.
